As a result of this growth, we are currently looking for someone to join our dynamic Management Team in the role of Nurse Manager.

As a member of the management team, you will work closely with the Director, along with other key internal and external partners, to grow the nursing program in the region.

The Nurse Manager will be responsible for the administration, organization and management of Community Nursing teams, ensuring compliance with quality care standards that are consistent with the organization’s core values, including patient-centeredness, excellence, partnership and accountability.

The Nursing Manager will also be responsible for operational and leadership functions, including meeting performance goals, onboarding and orienting new nurses, and ongoing mentoring, teaching, and managing team performance.

These responsibilities will be performed in accordance with the company’s policies, procedures, mission, vision and applicable legislation.

During the growth phase of the program, the Nurse Manager will serve in the capacity of work manager. This will include providing up to 20 nursing appointments per week, whilst supporting current nurses and increasing the nursing team to sustainably support the community.

This is a full-time position. Working hours will be from Monday to Friday, as well as participation in some On-Call work, which includes nights and weekends on a rotating basis.

What the role involves

Our leadership team plays a critical role in providing high-quality patient care, guiding our healthcare teams and inspiring others.

      • Provide professional and clinical leadership to direct reporting nursing staff (visiting and shift nursing)
        Serve as an ambassador for CarePartners, consistently representing the organization with professionalism, tact, and alignment with its core values
      • As necessary, act as liaison between staff and the College of Nurses of Ontario regarding standards of practice, etc.
      • Work closely with others, both internally and externally, as well as independently
      • Build strong teams that consistently deliver excellent, patient- and caregiver-centered care based on evidence-based clinical practice and in a fiscally responsible manner
      • Translate strategic objectives into practical actions that are sustainable
      • Conducting objective and constructive performance assessments, including clinical reviews as scheduled
      • Participate in 24/7 manager call rotation for assigned geographic area, carrying a pager and responding to calls promptly and appropriately
      • Identify, monitor and resolve issues around clinical and operational issues that impact the patient and caregiver experience;

adopts a quality perspective to identify and implement evidence-based, innovative and sustainable solutions for the program

      • Be actively involved with all safety and quality issues within the branch and agency
      • Promote and actively participate in the agency’s Continuous Quality Improvement Program and Risk Management Program
      • In collaboration with the Director, is responsible for the short and long term planning of the branch
      • In collaboration with the Director, seek opportunities to support integrated care, working closely with internal team members (i.e., Personal Support Managers) and external partners (i.e., local Ontario Health Teams) to provide a seamless experience for patients and caregivers
      • Demonstrate flexibility and ability to carry out long-term projects, as well as deal with daily problems and crises
      • Perform other related work as assigned

What do you bring

      • Registered Nurse (RN) / Registered Practical Nurse (RPN) or degree or diploma
      • 3 – 5 years of professional experience or an equivalent combination of education and experience
      • Registration with the College of Nurses of Ontario (CNO)
      • Experience in a community health environment considered an asset
      • Experience in a formal leadership role considered an advantage
      • Proven computer skills including email, word processing, work scheduling systems
      • In-depth knowledge of case management skills and nursing processes and practices
      • Vehicle is necessary and some regular trips are necessary
      • Leadership and people management skills are an advantage
      • Excellent verbal and written communication skills in English
      • Knowledge of the French language is an advantage
      • Current CPR Certification
      • Clear Background and Vulnerable Sector Check
